Collecting Your Scholarship Award

Congratulations on your scholarship! You must submit several documents before you may claim your payment. Scholarship awards are generally made in one installment. However, if the total award is equal to or greater than $3,000, your award will be divided into multiple payments. Please see below for the required documentation.

Scholarship Awards $3,000 and below

Scholarship awards of $3,000 and below are made payable in one installment. Scholarship checks are made co-payable to both the student and the school and will be mailed directly to the school's financial aid office. To claim your award, you must submit the following:

  1. Completed and signed Scholarship Award Agreement [PDF]
  2. Address confirmation form [PDF]
  3. Valid proof of enrollment [PDF]

Scholarship Awards above $3,000

Scholarship awards over $3,000 are divided into two installments (see your Award Agreement for details). Scholarship checks are made co-payable to both the student and the school and will be mailed directly to the school's financial aid office. To claim your first installment you must submit:

  2. Address confirmation form [PDF]
  3. Valid proof of enrollment [PDF]

Each subsequent installment will occur upon your successful completion of a quarter/semester at your school until your award has been expended. To claim each subsequent installment, you must submit:
